Occupational Excellence Achievement Award Application
Thank you for you interest in the National Safety Council Occupational Excellence Achievement Award.

The National Safety Council Occupational Excellence Award recognizes participants
(companies, units and/or facilities) that have reported injuries and illnesses that
involved days away from work, equal to or less than 50% of the BLS rate for their
6-digit NAICS code, and have had no fatalities during a calendar year, January
1 through December 31, 2007.
Criteria
Participants must demonstrate, through self-reported data, a total lost workday
case incidence rate (Number of cases with injuries and illnesses that involved days
way from work x 200,000/Employee Hours Worked) equal to or less than 50% of the
BLS rating for reported 6-digit NAICS code and have had no fatalities during a calendar
year, January 1 through December 31. Any organization recording a fatality will
not be considered.
Participants must submit an application by April 30, 2008. All information provided
is confidential and valid for a calendar year (January 1 through December 31).
Due to the nature of this program, we recommend that a Safety Representative/Manager
read and complete the following application.
Recognition
Participants that qualify for the National Safety Council Occupational Excellence
Achievement Award will automatically receive one free 8 x 10 personalized certificate.
Your application will be evaluated shortly after receipt. If your company, unit
or facility is deemed eligible based on your self-reported data, your award will
be mailed within 4-6 weeks to the shipping address indicated on your order form.
The National Safety Council reserves the right to change, without notice, any statement
in the Occupational Awards Program concerning, but not limited to, rules, policies,
awards, delivery dates/methods, and fees.
